Request An Appointment
Please use this form to request an appointment for the Partial Hospitalization Program (PHP) or Intensive Outpatient Program (IOP). Only submit this form if you are a new patient or are making an appointment for a patient.
After you submit your request, a UCI Health representative will contact you by phone within one business day to assist in making an appointment.
