Advance directive class

The best way to learn how to fill out an advance directive is to attend a class.

Our advance directive class is held every other month, and it's free for UCI Health patients and their family members.

This class is an opportunity to ask questions as you make decisions and fill out your own advance directive. Download an advance directive (PDF) ›

A notary will be on hand for those who would like their completed advance healthcare directives notarized.
