Health Assessment Program for Seniors (HAPS)

If you have complex, chronic conditions and are seeking a comprehensive consultation, the Health Assessment Program for Seniors (HAPS) can help you get on the path to a healthier, happier life. 

This unique consultation — the only one of its kind in Orange County — takes place in two parts:

  • First, you will meet with our team, which includes a geriatrician, neuropsychologist, social worker, pharmacist, occupational therapist and dietitian. During this meeting, our team reviews medical records, prescriptions, living situation and more in order to identify issues you may be having.
  • Approximately two to three weeks after the initial meeting, we will meet with you and your family to review our recommendations. These recommendations can be shared with your healthcare provider.

Our ultimate goal is to help older adults live more productive, healthy lives and to help families negotiate the difficult tasks of providing for care.

  • HAPS is a unique two-part consultation; it is the only one in Orange County and it serves as the model for similar programs around the nation.
  • We take a caring, individualized approach to patient care, based on a comprehensive evaluation of the patient by a physician who specializes in senior care (geriatrician), as well as social workers, neuropsychologists, pharmacists and occupational therapists who are experienced with the needs of seniors.
  • The approach of HAPS is a unique and effective model of care that involves the patient and family members in all discussions and decisions.
  • Due to the comprehensive nature of the HAPS consultation, the dialogue that takes place and the inclusion of the family in discussions, many patients and their families have said this experience is life-changing.
  • Once your consultation with the HAPS team is complete, we send our recommendations to your healthcare provider for further discussion and problem-solving.
HAPS is a consultation that is designed to identify problems and assist seniors who are experiencing the physical and mental challenges associated with aging, including:
  • Balance disorders
  • Concern about multiple medications
  • Memory problems
  • Dizziness
  • Falls
  • Living arrangements and independence
  • Incontinence
  • Physical decline
The first part of the consultation is a thorough assessment of your health that includes:
  • Physical exam
  • Living situation evaluation
  • Psychological assessment
  • Social assessment
  • Medication review

The second part of the consultation takes place approximately two to three weeks later, when a physician's assistant and attending geropsychologist present the findings and offer recommendations to the patient and family members. This may include recommendations for medications, ways to improve function, caregiving advice and occupational therapy.

After your consultation is complete, the team's recommendations are sent to your healthcare provider for further discussion and action.

The healthcare professionals with SeniorHealth Services believe that older adults with complex medical, psychological and social challenges need the help of a team of healthcare professionals in order to achieve the most comprehensive care plan possible.

Our HAPS team includes a geriatrician, psychologist, physician assistant, geriatric pharmacist, occupational therapist and a social worker. By working together, this experienced team of professionals is able to identify underlying issues that simply cannot be addressed in a standard office visit.
