Frequently asked questions

Cancer and its treatments can impact your fertility. While cancer itself may not directly cause infertility, treatments like chemotherapy, radiation and surgery can affect your ovaries or hormone production. Chemotherapy and radiation, especially near the ovaries or brain, can damage eggs or hormone-regulating areas. This can lead to a loss of your menstrual cycle or early menopause.

Your oncologist can help protect your fertility during surgery or cancer treatment in the following ways:


Ovarian shielding

Before radiation, your doctor can shield your ovaries from radiation to minimize exposure. 


Dose fractionation

Your oncologist will carefully control the radiation dose to kill cancer while reducing harm to nearby healthy tissues, including the ovaries.


Ovarian transposition

Surgeons can move your ovaries away from radiation areas to protect them. Recovery typically takes two days to two weeks, allowing you to start cancer treatment soon after.


It's also important for you to be involved in decisions about your fertility. Be sure to ask your doctor the following questions:

  • Will my cancer treatments affect my fertility?
  • Are there treatment options that won’t harm my fertility?
  • What fertility preservation options do I have?
  • How much time do I have before treatment starts?
  • Could my treatment cause early menopause or hormone issues? How can I manage them?
  • Is pregnancy safe after treatment?
  • What are the risks to future children?
  • Can you refer me to a fertility specialist?
If your menstrual cycle returns, it usually takes six months to a year, and occasionally longer. If your cycle has not returned or is very irregular more than a year after treatment, you may want to consider fertility testing.

Many women resume menstruation after cancer and are fertile. Others resume menstruation and are infertile. 

Producing and releasing normal eggs is an essential component of fertility potential. It is important to remember that other abnormal conditions, such as hormone imbalances, can also lead to infertility.

What to expect with fertility preservation

At UCI Health, we offer specialized fertility preservation options for patients undergoing cancer treatments that may affect fertility. 

Here’s what you can expect based on your diagnosis:

Cervical cancer:  For patients with invasive cervical cancer, a procedure called radical trachelectomy may help preserve fertility. It removes the cancer while leaving the uterus intact. Our gynecologic oncologists perform this operation with the da Vinci Surgical System®, which allows for faster recovery with less pain.  We determine your eligibility through careful evaluation.

Endometrial cancer: Some patients with endometrial cancer may be treated with hormone therapy to preserve their uterus. This decision requires careful discussion with our team.

Ovarian cancer: In some cases, we can remove only the cancerous ovary while preserving the uterus and the remaining ovary. Many women go on to have successful pregnancies.

Vaginal agenesis:  If your vagina didn’t develop normally, we offer surgery to create a vaginal canal. This may allow for sexual function and, if you have a uterus, pregnancy. If not, you can use your eggs for IVF with a surrogate.

Non-gynecologic cancer: 
If you’re undergoing chemotherapy or radiation for lymphoma or breast cancer, we can reposition your ovaries to preserve fertility. We can also protect them with medication.

What happens after my fertility preservation

If you’ve undergone surgical procedures, such as ovarian tissue freezing or robotic-assisted surgery, follow-up care will involve assessing recovery, managing any side effects and ensuring fertility options remain viable.
