About us

The UCI Health Family Health Centers, with locations in Anaheim and Santa Ana, is the oldest Federally Qualified Health Center (FQHC) in Orange County. At UCI Health Family Health Center — Anaheim, our goal is to improve the health and well-being of our patients by providing high-quality, accessible and comprehensive primary care to every member of the family, from newborns and children to teens and older adults. We have a long-standing history of delivering innovative care through group medical visits and providing integrative health therapies and treatments, including acupuncture and mindfulness training programs, as well as a teaching kitchen. It takes an extraordinary team to provide this level of care and we look forward to serving you.

We are affiliated with UCI School of Medicine and the Sue & Bill Gross School of Nursing at UCI. For over 40 years, we have trained medical students, nurse practitioners, physician assistants and resident physicians in family medicine, pediatrics and obstetrics and gynecology.

Referrals:

Depending on your health insurance coverage, we may be able to help you make an appointment with a specialist. To avoid delays, please familiarize yourself with your insurance plan.

Insurance & billing:

UCI Family Health Center — Anaheim accepts many insurance plans. Depending on your plan, you may be asked for a co-payment at the time of your office visit. At each visit, please provide a copy of your current health insurance card(s) or your current coverage eligibility letter. If you have billing questions, call 714-456-6324. Please tell us if you change your address or home or work telephone numbers.

Sliding fee scale

A sliding fee scale is available for patients and may offer a discount on services. It sets a discount based on the size of your family and your household annual income. Your level of discount is based on the Federal Poverty Guidelines established annually by the U.S. Department of Health and Human Services. All patients, regardless of their ability to pay, are able to be seen at UCI Health Family Health Centers on a sliding fee scale.

After-hours care & emergencies:

We have a physician on call after hours and on weekends for urgent questions. Call us at 657-282-6356.
